PHOENIX (3TV/CBS 5) - Every week, CBS 5 News is sent a list of restaurants that have been recently inspected by Maricopa County's Environmental Services Department. CBS 5 News selects restaurants with some of the highest numbers of "risk factors."

According to health inspectors, risk factors are considered major health code violations.

Among the violations:

Employee handled raw chicken then didn’t wash hands

Sour cream and butter not kept at proper temperature

Among the violations:

Raw eggs placed over vegetables

Crab meat and seafood salad not properly date marked


Wicked Six Bar and Grill

Among the violations:

Chicken kept past its discard date

Excess food debris on a slicer


Albertsons

6 violations

Among the violations:

Employee on phone – then not washing hands

Ham and cheese in deli case, past expiration date


Pakwan Indian Bistro

6045 W. Chandler Blvd

Chandler

6 violations

Among the violations:

Vegetables stored in grocery bags.

Personal medication near cooking equipment.

Heavy cream sitting out - had to be thrown out.
