According to health inspectors, risk factors are considered major health code violations.

Employee scooping chips with bare hands

4 violations

Among the violations:

Raw beef stored above cooked pork

Cook handling lettuce and onions without washing hands

2803 E. Broadway Road, Phoenix

5 violations

Among the violations:

Bread stored in grocery bags

Chicken wings not held at proper temperature

No soap at hand wash sink
