MARICOPA, Ariz. – The Maricopa Police Department has released the names of an officer and a sergeant involved in a fatal shooting Sunday afternoon.

Officer Joshua Hawksworth has been with the MPD since 2013 and Sgt. Leonard Perez has been with the department since 2007.

They are on paid administrative leave while the Department of Public Safety investigates the shooting of Johnathan Guillory, 32.

Guillory was shot during a confrontation with police outside his home near Cobblestone Farms Drive and state Route 347.

Police were doing a welfare check after receiving several 911 calls from the home. Guillory reportedly pulled a weapon on them and they felt their lives were in danger, according to police spokesman Ricardo Alvarado.

Hawksworth and Perez fired their guns.

Guillory was pronounced dead at Chandler Regional Medical Center.

Guillory was an Arizona military veteran. His widow said he struggled with post-traumatic stress brought on by a combat deployment in Iraq. He also spent time as a contract worker in Afghanistan.

Police had been called to the home several times by neighbors reporting disturbances.
