Texas man cited for causing major Prescott accident

PRESCOTT, Ariz. -- The driver of a pickup truck who caused a major accident in Prescott last week has been cited for multiple safety violations.

Samuel Fairfield, of Beaumont, Texas, was issued citations for exceeding the maximum length for a trailer combination, failing to control speed to avoid an accident and no proof of insurance.

Police said Fairfield was driving his truck, which was pulling a fifth-wheel trailer and a second trailer with a motorcycle on it, on state Route 69 at approximately 11 a.m. on July 31.

Fairfield attempted to turn onto Fain Road too quickly and was not able to negotiate the turn. He crashed his truck over the raised median, striking at least three vehicles as his overturned.

A Dodge minivan was pinned underneath one of the fuel tankers and the front of a Chevrolet Impala was struck.

Everyone survived the crash, though the damage to the vehicles was heavy.
