PHOENIX -- A fatal crash involving a motor home led to some major traffic restrictions on the Loop 202, in both directions, at 32nd Street.

Emergency crews responded to the scene on Monday afternoon. The driver of the motor home was killed, three adults and three children were injured, according to officers with the Department of Public Safety.

Investigators say the motor home was going westbound on the Loop 202 when, apparently, the left front tire blew out.

That caused the RV to hit the median wall between the westbound and eastbound lanes of the Loop 202.

When the RV struck the wall, it hit a light pole which caused the driver of the vehicle to be ejected. He was thrown into the eastbound lanes of the Loop 202 but was not struck by any other vehicles.

The drivers was Lee Holt, 37, of Buckeye, Ariz. Holt was wearing his seat belt, according to DPS officers.

The driver's side of the RV was ripped off as it traveled approximately 200 feet along the wall.

There were three children in the RV, ages 9, 11, and 12.

The nine-year-old child was transported to Phoenix Children's Hospital with serious injuries. The other victims were taken to Maricopa County Medical Center with various injuries.

No other vehicles were involved in the crash. One vehicle did get a flat tire from driving through the debris.

The RV was towing an SUV.

Officers shut down the eastbound and westbound HOV lanes. The incident caused some major traffic backups in both directions on Monday afternoon.

The crash scene is expected to be cleared and all lanes reopened before rush hour.

Passenger / Hayden Holt, 9, serious injuries and was transported to Phoenix Childrens Hospital
Passenger / Lea Pena, 60, serious injuries and was transported to Maricopa County Medical Center
Passenger / Paul Pena, 66, non-life threatening and was transported to Maricopa County Medical Center
Passenger / Noah Holt, 12, minor injuries and was transported to Maricopa County Medical Center
Passenger / Ryan Holt, 11, Serious injury and was transported to Maricopa County Medical Center
