Arrowhead Hospital earned a top grade based on preventable medical errors, injuries accidents, and infections.
The 217-bed acute care facility, an affiliate of Abrazo Health Care, also captured an "A" score in 2012.
The Hospital Safety Score is designed to give the public information they can use to protect themselves and their families.
The hospital is located south of Loop 101 at 67th Avenue and Union Hills.
The Leapfrog Group is run by employers and other large purchasers of health benefits.
