Arizona's Family Spirit Award

The Arizona's Family Spirit Award recognizes individuals and/or small businesses for performing acts of goodwill toward others that strengthens the community and improve lives.
And remember, someone can be a volunteer without being affiliated with a nonprofit organization. It is often the caring things we do on a daily basis in our interactions with others that give a city a sense of community. After all, that's what 3TV Arizona's Family is all about.

Arizona's Family Spirit Award Official Rules
1. ELIGIBILITY: No purchase necessary to enter. Individuals and/or small businesses within the 3TV viewing area are eligible. The Arizona's Family Spirit Award will recognize an individual every other week in the 3TV viewing area May through December 2006 for performing an act or acts of goodwill toward others in order to strengthen the community and improve lives. Employees, temporary employees, independent contractors and interns of 3TV, azfamily.com, Belo Corp., their respective subsidiaries, parents, partners, other contest sponsors, advertising and promotional agencies and the immediate family members of each, are not eligible.
2. RULES AND NOMINATION FORMS: Official rules and entry forms are available on azfamily.com, at 5555 North 7th Avenue, Phoenix, Arizona 85013, by sending a stamped, self-addressed envelope to that address Attention: Arizona's Family Spirit Award, or by calling 602-207-3337. Deadline for nominations for 2006 is December 1st. All entries become the property of 3TV and azfamily.com. No entries will be returned. One may nominate more than one individual; however, each nomination must be submitted on a separate form. No responsibility is assumed for lost, misdirected or late mail. Recipients will be selected at the sole discretion of a panel of judges selected by 3TV and azfamily.com. The judges' decisions are final.
3. PRIZES: Each person and/or small business selected will be featured on 3TV and azfamily.com. Recipients may be required to execute an affidavit of eligibility and release form. All recipients must present proper identification. No alterations or substitutions for prizes will be permitted. All decisions are final and will be based on the judgment of the sponsors. This offer is void wherever prohibited and subject to all federal, state and local laws. Taxes on prizes are the sole responsibility of the recipients.
4. USE OF CONTEST INFORMATION: Contestants grant to Contest Providers, and Contest Providers reserve, the right to use any and all information related to the Contest, including information provided by Contestants in connection with the Contest, for marketing purposes or any other purpose, unless prohibited by law. Contestants may be required to sign a release, which grants to Contest Providers the right to use and publish their proper name and state of residence online or in any other media in connection with the Contest. By accepting a prize, winner(s) may be required to sign a release that grants permission to Contest Providers to use his or her name, photograph, likeness, voice, prize information and/or biographical information for publicity and promotional purposes without future compensation unless prohibited by law. By entering, participants release and hold harmless Contest Providers, their respective parents, subsidiaries, affiliates, directors, officers, employees, and agents from any and all liability for any injuries, loss or damages of any kind arising from or in connection with the Contest or any prize won. Restrictions, conditions and limitations may apply.
5. LIMITATIONS OF LIABILITY: Winners assume liability for losses or injuries caused or claimed to be caused by participating in the Contest, or by the acceptance, possession, use of, or failure to receive any prize. Contest Providers are not responsible: (i) for any lost, late, illegible, incomplete, postage-due, misdirected or mutilated mail; or (ii) for any incorrect or inaccurate information, in the Contest or by any technical or human error that may occur in the processing of submissions in the Contest. Contest Providers assume no responsibility for an error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft or destruction of authorized access to, or alteration of, entries. If the Contest is not capable of running as planned for any reason, including legal restrictions, tampering, unauthorized intervention, fraud, or any other causes beyond the control of Contest Providers that corrupt or affect the administration, security, fairness, integrity or proper conduct of the Contest, Contest Providers reserve the right to cancel, terminate, modify or suspend the Contest.
6. GENERAL CONDITIONS: This Contest is subject to all federal, state, and local laws and regulations and is void where prohibited by law.
