Silver Apple Award Official Rules

1. No purchase necessary to enter.

2. Any student enrolled in kindergarten through 12th grade in an Arizona school may enter by including their name,
address, phone, the names of their parent(s)/guardian and the teacher being nominated. The nominator must state in 100
words or less "Why This Teacher Is Special." The entry must be signed by the school's administrator (not the teacher)
and mailed to: Silver Apple Award, 5555 North 7th Avenue, Phoenix, Arizona 85013. One student may nominate more
than one teacher; however, each nomination must be mailed on a separate form. A supporting letter (50 words or less) by a
parent or administrator can be included. Illegible, mutilated, incomplete or duplicate entries will automatically be
disqualified. A total of 8 awards will be presented during the 2007-2008 school year.

3. The program will run from August 6, 2007 through May 30, 2008. Entries must be received by
March 3, 2008 at 5:00 p.m. No responsibility is assumed for lost, misdirected or late mail. Recipients will
be selected at the sole discretion of a panel of judges selected by 3TV, Arizona’s Family and azfamily.com. The
judges' decisions are final.

4. By entering or winning this contest, each entrant and award recipient agrees that 3TV, Arizona’s Family and azfamily.com
and their affiliates may use their names, likenesses, entry stories and additional information concerning the entrant and
award winner in any promotional activities relating to this contest without further compensation to the winners. 3TV,
Arizona’s Family and azfamily.com assume no responsibility for any loss, claims, damages or injuries resulting from
winning prizes.

5. Eight teachers (October-May) will be chosen as recipients and each will receive a Silver Apple Award and $500 to be used
toward classroom projects or materials.

6. Only one student nominator, per nomination form, will be accepted. Multiple students may nominate the same teacher, but
each nomination must be an individual's own essay, and each nomination will be treated individually by the judges.

7. The recipients will be announced on 3TV Arizona’s Family newscasts during the program period. Recipients may be
required to execute an affidavit of eligibility and release form. All recipients must present proper identification. No
substitution for the prizes nor any alterations will be permitted. All decisions are final and will be based on the judgment of
the sponsors.

8. Nominations may be made by any Arizona student except for those whose parent(s) are employees, temporary employees,
independent contractors or interns of 3TV, Arizona’s Family, azfamily.com, their affiliates, subsidiaries and advertising
agencies. This offer is void wherever prohibited and subject to all federal, state and local laws. Taxes on prizes are the sole
responsibility of the recipients.

9. For a list of recipients, send a stamped, self-addressed envelope to: Silver Apple Award Recipients, 5555 North 7th Avenue,
Phoenix, Arizona 85013.

10. Official rules and entry forms are available by sending a stamped, self-addressed envelope to: Silver Apple
Rules, 5555 North 7th Avenue, Phoenix, Arizona 85013.The application is also available on the internet at
www.azfamily.com/silverapple. All entries become the property of 3TV, Arizona’s Family,
and azfamily.com. No entries will be returned.
